Update Sleep
The Update Sleep Form enables you to enter data regarding your night
Morning TAB
After waking up in the morning, begin by entering your data in the Morning tab.
Both the date you went to bed and woke up will be automatically filled in for you.
How long did it take you to fall asleep? This is also known as Sleep Onset.
Enter the time you got into bed - Make a note of this time before actually getting into bed
Enter the time you woke up - Enter the time you recall waking up
Night Sweats - Check the box and select the severity.
Restless Legs - Check the box and select the severity.
Snoring - Check the box and select the severity
Questions -
Effort to sleep - Select from the three choices
Sleep Quality - Select from the choices listed
Outcome - Select from the three choices
Doze off potential - Select from the four choices
EEG Device - Check this box if you own a Muse-S. The EEG Data TAB will appear
This is the best way of knowing how well you sleep. It is probably the most accurate system available outside of a sleep clinic.
If you own a Muse EEG headband, you can enter the data from your phone right into this area:

Muse TAB
The Muse tab displays all the bands you've purchased since using the Muse-S device. This is important because when your band starts to wear out, you'll be asked to supply the serial number of the band you're wearing.
The Awakenings listbox is there for you to enter each time you woke up during the night
Event - Displays a description of what happened to wake you up
Activity - Displays if you carried on an activity such as getting up and reading, going to the bathroom, eating some food, etc.
OOB - Displays if you were Out of bed
Date - Displays the date (since sleep usually starts on one day and ends on the next)
Time - Displays the time of the event
Start - Displays the beginning of an event. You might wake up at 1:00am, lie there for 4 minutes, then decide to get up.
End - Displays the time you got back into bed and are attempting to go back to sleep
S.O. - Time it took you to go back to sleep after an event
Dur - The total time you were awake during that event
Evening Tab
The second tab labelled Evening enables you to enter data on how you spent your day, if you exercised, drank caffeine or ate a lot of food prior to bed
At the end of the day, you'll want to fill out any area that pertains to your activities
Caffeine - Check the box if you recall eating or drinking something with caffeine.
Exercise - Check the box if you were able to exercise for at least 20 minutes during the day
Naps - Check the box if you found it necessary to lie down for a period of time during the day
Mood - Fill out the two statements regarding how you felt emotionally during the day
Bedtime - Check any of the boxes that apply to you (Alcohol, eating, caffeine)
If you were sitting down and just dozed off, check the box
Set Day Rating Same As Sleep Rating - Checking this box will apply the sleep rating score to the Day Rating score
Daily Bedtime Routine - Check this button

There are two sleep ratings you can fill out. One for rating your night and another for rating your day
