Update Medication Identity
 
This form enables you to make changes to any of the records in the Medication Identity table. For the most part, all the information you'll need will already be entered. If you find more information about a particular tablet manufacturer, you'll be able to enter that information, such as the inactive ingredients.
 
Medication Shapes Tab
The Medication Shapes Tab contains an image of what many of the tablets' shapes look like.

Medication Imprint: Enter the tablet/capsule ID number
Medication Color: Enter the color of the tablet/capsule
Medication Shape: Enter the dimension of the tablet/capsule
Manufacturer: Select the name of the manufacturer
Medication: Enter the name and strength of the thyroid medication
NDC Number: Enter the National Drug Code of the thyroid medication (Optional)
Combo: Select this checkbox if the thyroid medication is a combination of T4 and T3 (Synthetic)
NDT: Select this checkbox if the thyroid medication is Natural Desiccated Thyroid extract
Discontinued: Select this checkbox is the thyroid medication is no longer available
Select File Image: You'll be able to access the image of the thyroid medication. Note: Not all thyroid medications have available images.
Why it's important to have an image - You should always be familiar with the ID number of the medication(s) you're taking. This can prevent
being injured. Many people die or are injured each year because a prescription was filled wrong and the patient just assumed that they received the right
medication. Never make this assumption! Always check! Never take a new medication before confirming it's the right drug!
 
Requirements for adding an image:
The image must be 500 X 500 pixels in size. It can be either a jpg or png format.
If our database doesn't contain your medication image and you would like us to format an image, send us an email. You can do so directly through the Thyroid Tracker Pro program.
