Update Allergy
The Update Allergy form enables you to add or make changes to records regarding any allergies you might have
Allergy - Enter the name of the allergy you have
Reaction Type - Select the region from where your allergy comes from
Reaction - Enter what happens when you're exposed to the allergen
Severity - Select how serious this reaction becomes
Notes - Enter any comments you believe are appropriate for this allergy
