Questionnaire - View Scores
The Questionnaires window enables you to complete and print out a report based on signs and symptoms of thyroid disease.
The questionnaire consists of 25 questions regarding thyroid signs and symptoms. Once you've filled out the questionnaire completely and saved it, you can return to edit it for up to 7 days afterwards. After that, you won't be able to change any of your answers. The reason for this is that relying on memory of past events or moments in time can be inaccurate.
You can take the questionnaire as many times as you want. However, this is just part 1; you'll want to review part 2 and complete those blood tests in order to get a comprehensive report.
